Medical Release Form

Each student must have an updated medical release form for any overnight or out of town event. These are available at the welcome desk on Sunday morning, at Grindstone on Wednesday night, or for print on our "Students" page of the website. Unfortunately, because of the necessity of a signature, Medical Release Forms are not available to fill out online.
